The Ethics of Using A Comprehensive, Medically Based Intake Form

This class is designed to bring a broader knowledge to the massage therapist about the necessary information that should be included on your Intake Form. Information that is necessary to the delivery of your massage. Is your client receiving any type of medical treatment? What medications and/or supplements are they taking and the side effects they
may be experiencing. How both the medication and side effects may alter how you massage that client. A sample medically based Client Intake Form will be shared.
Different dialoging techniques with your client when reviewing the Intake Form will be discussed. These techniques allow for more detailed information about the client’s symptoms to be gained thereby allowing for a more personalized approach to working with the client’s symptoms.
**This class is approved by the S4OM as a supplemental class toward the specialty certification in oncology massage now offered by the NCBTMB

Cancellation and Refund Policy: Student will be refunded 100% if class is cancelled by instructor. If student voluntarily withdrawals from a class 30 days or more prior to class date, he/she will receive a 100% refund (PayPal or credit card fees may apply). If withdrawal occurs 15-29 days in advance of the class date, student will receive a 50% refund (PayPal or credit card fees may apply). Student will not receive a refund if withdrawal occurs 14 days or less prior to scheduled class. A $25 rescheduling fee may apply. Special exceptions may apply. Classes are not guaranteed to take place and could be cancelled due to low enrollment, weather, etc. Please check with Sage before you make plans or hotel or travel arrangements.
